Following  a National Government initiative from 1st September 2018 extra appointments will be offered across the whole of East Staffordshire, including evening and weekend appointments. Additionally a new on line digital service will be available on Sunday mornings where appointments will be offered with a GP via the Q Doctor App. Posters will be available in the surgery shortly; please click on this link for further details https://www.qdoctor.io/

Registration as a New Patient

 



 

If you live in our practice area and would like to register with us, please complete one of our registration forms that are available from reception. You will be asked to sign this form to confirm that the details are correct.

When you register you will also be asked to fill out a medical questionnaire this is because it can take a considerable time for us to receive your medical records. Please bring with you a list of all your regular medication; a prescription counterfoil would be ideal.

 

Please remember that photographic identification will be required for all new patients completing regisration at our surgery along with a utility bill or bank statement confirming your address.

 

You will be registering with the practice rather than an individual GP. You may request which GP you would like to see when booking an appointment.

 

When you register as a new patient with us, we activate your online patient access account, we will then email you this information (if you have left us a valid email address) which you can then use to sign up. To find out more about online services please click here.
