Following  a National Government initiative from 1st September 2018 extra appointments will be offered across the whole of East Staffordshire, including evening and weekend appointments. Additionally a new on line digital service will be available on Sunday mornings where appointments will be offered with a GP via the Q Doctor App. Posters will be available in the surgery shortly; please click on this link for further details https://www.qdoctor.io/

All practices across East Staffordshire are participating in this extended access. For further information please contact the surgery reception.

Registration as a New Patient

 



 

If you live in our practice area and would like to register with us, please click onto the next tab above 'Registering Online' and complete the electronic 'Pre-Registration Form'.

You will also need to complete the 'Online Medical Questionnaire For New Patients' form.  

This is because it can take a considerable time for us to receive your medical records and providing us with this important information about your medical history and medications will enable us to ensure a smooth transfer of services from your previous GP practice.

 

Please remember that we MAY ask for photographic identification and/or proof of address for Patient Online Services - for more information about Patient Online Services please see below:

 

When you register as a new patient with us, we activate your online patient access account, we will then email you this information (if you have left us a valid email address) which you can then use to sign up. To find out more about online services please click here.
