Some services are not covered by the NHS and we will therefore charge a fee for completion of this work. The NHS medical care of our patients takes priority during our working day so any private letters or forms can take up to 8 weeks to complete and return to you. We cannot guarantee they will be completed earlier.
A list of our current fees and methods of payment with effect from 1st April 2019 (to be reviewed again in April 2020) is provided below:
Medical examinations
- HGV/LGV/PCV/coach/bus/taxi – £120.00
- Elderly driver report (without examination) – £104.00
- Elderly driver examination and report £110.00
- Pre-employment examination and report – £110.00
Medical reports ( no examination required)
- Life insurance report – £104.00
- Medical/occupational/solicitor/firearms report – £104.00
Miscellaneous
- Dispensing fee for private prescription – £10.00
- Simple letter / certificate – £20.00
- Complex letter / certificate – £35.00
- Driving license application – £20.00
- Private sick note – £18.00
- More complex private sick note – £35.00
- Childminder ofsted health declaration form – £91.00
Adoption and fostering
Adoption fees are usually paid by the prospective parent, sometimes by the adoption / fostering agency or authority. This needs to be established prior to the appointment.
- Form AH – health assessment, prospective carer – Price is set externally
- Form AH2 – adult health update, parent – Price is set externally
Travel
Some vaccinations required for travel purposes carry a fee. Once a travel questionnaire has been completed and our practice nurse has made her recommendations, the cost of your vaccinations / treatment will be quoted.
- Fitness to travel letter/certificate – £20.00
- Holiday cancellation letter/form ( simple document 1 page) – £20.00
- Holiday cancellation letter/form ( simple document 2 or more pages) – £60.00
- Overseas visitor private consultation ( cost of drugs and dressings plus VAT will be additional) – £54.00
Method of payment
We accept cash and cheques only. Cheques should be made payable to ‘Northgate Surgery’ unless advised otherwise by the receptionist or doctor.
Payment for services provided to patients not registered here must be in cash only.
Payment must be made at the beginning of a course of treatment/vaccinations.
